Google Forms: How to create a form
Creating a google form is pretty straightforward, you can follow these steps.
Step 1: Visit docs.google.com/forms.
Step 2: Just hover over the + icon and start creating a new form or you could choose a template. If you want to start from scratch, hit Create new form.
Step 3: Then you can add the title and the description.
Step 4: The questions could be added in the box below. If you want to keep adding more questions, you have to just hit the + icon from the toolbar.
Step 5: You can also import questions from other forms, add a description, subtitles, photos, etc.
Step 6: You can always see how your form looks by clicking on the Preview button on the top right, next to Settings.
Google Forms Customisation: How to design forms
Here is how you can design your forms.
Step 1: First, hit the Customize Theme icon to open the theme options.
Step 2: You can then choose a preloaded image or a personal photo as the header.
Step 3: Then you can set the theme colour of the header image according to your liking. You have to keep in mind that the background colour depends on the Theme colour that you choose.
Step 4: Finally, you can choose from any of the different font styles available.
Google Forms: Field options
When you create a form, you get a bunch of field options along with it.

You can choose how you want others to answer your questions after writing them.

There are different options like, Short Answer, which is for one line answers and Paragraph, which asks for a more detailed answer.

You can also set the answer to be Checkboxes, Dropdown or Multiple Choice.

If you want the respondents to choose from lower to higher options, even that can be set by selecting the Linear scale for answering.

The respondents could also be asked to answer in the form of adding files, which can be photos, documents, videos, etc. You can also set the maximum number of files and the maximum size of the files.

You can also choose Data and Time if your question demands asking the specific date and time.

You can also create a duplicate field by hitting Duplicate and you can remove a field by hitting Delete.
Google Forms: How to create a quiz
You can create a basic form by following the instructions above but if you want to create a quiz, you can follow the following steps.

Go to Settings, then hit the Quizzes tab and enable Make this a quiz.

According to your will, you can decide whether to give the answers immediately to the respondents or declare them manually later.

Then you can also choose what your respondent sees in the form of Missed questions, Point values and Correct answers. Then, hit Save to close.

Now, you need to give the correct answers to the questions and assign the points for each. For this hit the Answer Key, then tick the correct answer, then assign the points and hit Save.

Now, as the respondents answer the questions correctly, they will be rewarded with the assigned points.
Google Forms: How to share responses
You have now learnt how to create a form, design it and then present it as a quiz or a survey. Now, let’s take a look at how you could collaborate with others in making your form and then share it.
Step 1: You can easily collaborate on your Google Form, you just need to hit the three dots icon and click the Add collaborators.
Step 2: Then the emails of the collaborators can be added, or you could just copy the link and share it with them through third-party apps like WhatsApp Web or Instagram, etc.
Google Forms: How to see responses
The google forms can be accessed on your Google Drive or you can even visit the Google Forms website and access them. You can follow these steps to evaluate a particular form.
Step 1: First, open the form that you would like to evaluate.
Step 2: Go to the Responses tab. Then you need to first disable the Accepting Responses tab so that the respondents cannot make any further changes to the form.
Step 3: You can also take a look at the Summary tab to see the performance of all the respondents.
Step 4: Then you can go to the Question tab which allows you to evaluate the responses one by one.
Step 5: And the Individual tab will let you evaluate the performance of each individual.
